The front has a sewn-in 1.65" O-ring (the round opening that holds your dildo in place) set into a built-in stabilizer center. That's a firmer panel stitched around the ring that keeps the fabric from stretching much, so your toy wobbles less.
There's also an interior pocket for a removable Neoprene Stabilizer, a soft, shield-shaped insert that adds extra support behind the O-ring and lets you use dildos with a narrower base. Pick the one that matches your favorite toy, measured across the base of the shaft:
